Community Health Navigation (CHN) Expansion
Project Description:
Austin Asian Community Health Initiative is dedicated to improving the health and well-being of Asians in Central Texas through advocacy, education, and support.
The grant is unrestricted and awarded in support of the mission and programs of AACHI.
Grant Status:
With the unrestricted grant to AACHI, they expanded their Community Health Navigation Program, which provides healthcare navigation for Austin’s vulnerable Asian subpopulations, including Burmese, Korean, Nepali, Vietnamese, Syrian, Iraqi, and other Arabic speaking communities. Services include health insurance enrollment; securing affordable housing; and applying for state benefits like SNAP.
Hired 3 additional Community Health Workers, for a total of 10, with services in 9 languages
Served 430 immigrant families, representing a 10% increase from the previous year
